Annual Visits to Patients Over 75 Years
We offer a yearly home visit service to all of our patients aged 75 years
and over by a Practice Nurse. The nurse will send out a letter with a date
and time for the visit, which will take about 30 minutes. Where appropriate,
the nurse will:
- Check your blood pressure
- Take a urine sample
- Take a blood sample (usually to monitor the effects of any medication
you are taking)
- Assess whether you would benefit from any other help, including
referral to Social Services for home help, etc.
- Refer you to other medical services, including chiropody,
physiotherapy, the Community Resource Team for Older People (CROP) or
District Nurses
- Make a general health assessment, including assessment for Falls or
Dizzy Spells
- Help with completing attendance allowance forms
- Make a Blue badge (parking) forms
- Ensure that your details (including next of kin) are up to date
- Liaise with other Healthcare professionals, including the Doctors.
